Clinical Coordinator – Home Health Agency
Full time: 9am-5pm est
Pay: $1300-1400 USD per month
We are seeking a Clinical Coordinator to support the day-to-day coordination of patient care for our home health agency. This person will serve as a key point of contact between patients, families, caregivers, and the agency, ensuring schedules are properly managed and everyone receives timely support and communication.
Key Responsibilities
- Coordinate and manage caregiver schedules and patient visits
- Communicate regularly with caregivers regarding schedules, changes, call-outs, and patient needs
- Speak with patients and family members to address questions, concerns, scheduling needs, and changes in care
- Find coverage when caregivers are unavailable and minimize gaps in patient care
- Follow up with caregivers to ensure scheduled visits are being completed
- Maintain accurate and up-to-date patient and caregiver information
- Communicate important updates or concerns to the appropriate clinical or management team
- Help resolve scheduling conflicts and caregiver/patient concerns in a professional and timely manner
- Maintain strong relationships with patients and families by providing excellent customer service
- Document calls, scheduling changes, and relevant patient/caregiver communications
- Assist with general administrative and coordination tasks as needed
Qualifications
- Previous experience in home health, home care, healthcare coordination, scheduling, or a similar role strongly preferred
- Excellent communication and customer service skills
- Comfortable communicating with patients, families, and caregivers throughout the day
- Strong organizational and multitasking skills
- Able to handle schedule changes and urgent situations calmly and efficiently
- Proactive and solution-oriented
- Strong attention to detail and follow-through
- Comfortable working in a fast-paced healthcare environment
